The following information will be required for you to set up your account:
 
- Donor I.D. Number
- First Name
- Last Name
- Date of Birth (mm/dd/yyyy)
 
You can obtain this information from your donor ID card or pick up a form from the registration tech at your next donation.

Exit onto I-71 North (49 B).  Take the Fields Ertel Rd. Exit (Exit 19) and turn left onto Mason-Montgomery Road.  Turn right into the Governor's Point Shopping Center.
