Hoxworth Blood Donor FAQs

Please call (513) 558-1304 if you have a reaction or illness following your donation or call 911 if you have a medical emergency. 

You will receive your Premier Donor Club rewards based on your donation frequency for your donation platform. You will receive an email from our rewards manager when you earn your rewards with a link to select an e-gift card from over 70 brand vendors within seven days from your donation that qualifies you for your reward. To protect your information, we require our donors to create an online portal through our website, Hoxworth.org to obtain information related to your donor record such as blood type, donor id and donation history.

3 Tips to Creating Your Donor Portal

  1. First & Last Name - Your donor portal must be set up using the same first and last name as you register for your donation. 
  2. Email - The email that you set up your donor portal must match the email listed on your donor record you register to donate.
  3. Donor ID - Your donor ID and Unit number is not required information to set up your donor portal. To ensure your portal and donation record sync, please enter the information as outlined in steps 2 and 3.

NOTE: We must match your donation record to the first and last name on the ID you present at the time of donation. Any deviations to your name and email will cause an error that will not allow your portal account to link with your donor record. This will prevent you from viewing your donor id, blood type and any information related to your donor record.
